You will need to bring:
- Your referral from a GP or medical specialist.
- Any scans or X-rays you may have had (the films and discs please, not just the reports)
- Any results of blood tests or other investigations.
- You will also need a list of your medications and allergies.
- Please remember to bring your Medicare or DVA card.
If you require an interpreter, please tell the secretary when making the appointment.
